ADA Complaint/Grievance Procedure
In order to process a complaint, a formal complaint must be filed within 180 days of the occurrence of the alleged discriminatory act. Assistance is available upon request by contacting Civil Rights at (501) 569-2297. All complaints received by ARDOT ADA Coordinator will be retained for five years internally then electronically stored.

Download, Print and complete the form by including all necessary information. The form is located here:  ADA Complaint Form and also available upon request. Once the form is completed, you can return the form to:   Arkansas Department of Transportation  Civil Rights Division Attn: Joanna P. McFadden, Civil Rights Officer (ADA/504 Coordinator) P.O. Box 2261, 10324 Interstate 30 Little Rock, AR 72209. Within 15 calendar days after receipt of the complaint, the department’s ADA Coordinator or their designee will meet or communicate with the complainant. The ADA Coordinator or their designee will respond in writing or in an alternative format accessible to the complainant. This response will explain the position of the department and offer options for substantive resolution of the complaint.

Procedimiento para Quejas y Reclamaciones

Para procesar una queja, se deberá presentar una queja formal dentro de los próximos 180 días de haber ocurrido el presunto acto discriminatorio. Existe la posibilidad de solicitar asistencia disponible poniéndose en contacto con la Divisisión de Derechos Civiles llamando al teléfono (501) 569-2297.

Todas las quejas recibidas por el Coordinador de ADA en ARDOT se conservarán durante cinco años internamente y luego se almacenarán electrónicamente.

Para iniciar el proceso, siga los siguientes pasos: Descargue, imprima y llene el formulario incluyendo toda la información necesaria. El formulario se encuentra en:  ADA Complaint Form y está disponible con previa solicitud. Una vez completado el formulario, puede enviarlo al Departamento de Transporte de Arkansas ARDOT División de Derechos Civiles Atención: Joanna P. McFadden, Oficial de Derechos Civiles (Coordinadora ADA/504) P.O. Box 2261, 10324 Interstate 30 Little Rock, AR 72209. Y debe hacerse en un plazo de 15 días hábiles o de oficina. Una vez que haya sido recibida sido dicha queja, el Coordinador de ADA del departamento, o la persona que haya sido designada, se reunirá o se comunicará con la persona demandante. El Coordinador de ADA o su designado responderá por escrito o de una forma accesible para el demandante.
